We email you a short registration and consent form. Please complete it before your visit — it tells us about your child and how you would like to be billed (self-pay or insurance).

If your insurer requires pre-authorisation, please have it in place before the appointment.

You see Dr Gray at HCA Chiswick Outpatients. Self-pay patients are invoiced by email after the appointment; insured patients are billed or reimbursed as described on our fees page.
To change or cancel, please call us on 020 3633 2915 (Monday–Friday, 09:00–18:00). We need 2 working days' notice, otherwise a £120 cancellation fee applies. See our fees page for details.
